Model Release

MODEL RELEASE 

For and in consideration of my engagement as a model by Richard C Hemingway, hereafter referred to as the photographer, on terms or fee hereinafter stated, I hereby give the photographer the right and permission to copyright and/or use, reuse and/or publish, and republish photographic pictures of me. The photographer may only use pictures that have been approved by me; such pictures will be listed below.  Pictures that I do not approve of will also be listed below.  I further acknowledge that approved pictures will have  background changes* and such changes such as color of clothes, lips, hair, eyes of model may change as deemed necessary by the photographer. Anything further than these types of changes I will have to approve (or legal guardian) for the picture to be used commercially. [*note: applies to such things as adding trees, grass, sand dunes, water, mountains, sunsets , buildings or anything that will add emphasis to the models picture.]

            I hereby waive any right to inspect or approve the finished photograph or advertising copy or printed matter that may be used in conjunction therewith or to the eventual use that it might be applied.  This provision only applies to pictures that I have already approved.

I am to be compensated as follows: 

Paid on day of shoot or jpegs of finished pictures or finished pictures sold for cost of material:

40% of net profits on each picture sold.

For models under eighteen: 

I hereby certify that I am the parent and/or guardian of                             a person under the age of eighteen, and in consideration of value received, the receipt of which is hereby acknowledged, I hereby consent that any approved photograph taken by the photographer, may be used by him for the purposed set forth in original release hereinabove, signed by the model, with the same force and effect as if executed by me.
